CareAtHome example support plan

This Organization does not sell a report. What the family keeps is the support plan: who visits, what the visit is for, the line between support and care, and how anything that goes wrong reaches the right person.

A complete sample confirmation, written the way CareAtHome writes one. The customer, the dates and the amounts are illustrative, and nothing here was charged or reserved.

Marija Novak visits Olga every Thursday from 10:00 for an hour. The same person every week, which is the whole point of the service. Clause 03 is the boundary, and it is firm in both directions: it protects Olga and it protects Marija.

Plan agreed, worker matched, consent recorded

Weekly companionship visit, Thursdays 10:00 to 11:00, from 8 October. Marija Novak, the same worker each week with a named cover. Olga consented to the plan herself on 29 September in Tadej's presence, and that consent is recorded because it is hers to give rather than her son's.

The person

What Olga wanted from this, in her words

From the assessment on 29 September, with Olga present and answering for herself: "I do not need looking after. I would like somebody to have coffee with who is not my son, and I would like to get to the cemetery on a Thursday without asking anyone for a favour."

AgreedDetail
The hour is Olga's She decides what it is used for each week, within the tasks below
Usually Coffee at the table, conversation, and the walk to the cemetery and back
Sometimes A short shop on the way back if she wants one
Not Cleaning, which she was firm about and does not want
Language Slovene
Her preference Marija rings the bell and waits. She does not want anyone letting themselves in

The last row is in the plan because Olga raised it unprompted, and it is the kind of thing that matters more to the person receiving care than almost anything else in this document.

02

The visit

What Marija does

  • Arrives at 10:00, rings, waits. Does not have a key and will not be given one without Olga's agreement
  • Companionship: conversation, coffee, company. This is the service, not a filler between tasks
  • Accompanies Olga out on foot, including the cemetery walk, and back
  • Light meal preparation if Olga wants it, in Olga's kitchen, to Olga's instruction
  • Carries the shopping, not the decisions
  • Writes a short visit note afterwards and sends it to Tadej
  • Leaves at 11:00, or stays a few minutes if a conversation is not finished
