Supporting Dad: four people, one week, no gaps
Who does what, when, and the three slots nobody was covering.
Four people are already helping and it is going wrong anyway, because none of you can see the whole week at once. This plan is the whole week on one page. It found three uncovered slots and two where two of you were duplicating each other, and it changes nothing about what anyone actually does.
Thursday afternoons, Sunday evenings and the Friday pharmacy run have nobody attached to them. Meanwhile Kees and Lotte are both calling on Tuesday and neither knew. Fixing both takes one conversation and no extra hours from anyone.

Marijke is a neighbour doing a favour, not a carer, and the plan treats her that way: short, predictable, morning only, with no task that would matter if she were away.

Four of the five are a person moving something they already do to a different day. The only new habit is the shared note, and it is the one that stops this plan going stale in a month.

One person owning the GP relationship matters more than it sounds. Professionals give clearer information to a single consistent contact, and it stops three siblings receiving three slightly different versions.
